| During the month of January, 2009 you should complete the OSHA Log 300 form covering work-related injuries and illnesses for the calendar year 2008. Verify the presence in file of a back-up document for each Log entry. This may be an OSHA Form 301 or a copy of the Workers Compensation report for the identical injury or illness. (This will eliminate duplication of data on separate forms as OSHA will accept the State Workers Compensation form.)
Before February 1st, complete the OSHA's FORM 300A - Summary of Work-Related Injuries and Illnesses. This Annual 2008 Report should be posted where all employees will have easy access. The form must be displayed from February 1st through April 30th. The form must also be signed by the highest ranking officer or manager on the premises. Falsification of the records could result in criminal charges against that management representative; especially if it is determined a known falsification was recorded. Files for three prior years must be maintained. If an incident occurred where continuing medical monitoring will be required annually, the record must be retained for 30 years.
